A comparative study between ultrasound guided and peripheral nerve stimulator guided brachial plexus block by supraclavicular approach

Abstract

Introduction: Successful peripheral nerve and plexus blockade (PNB) can provide an excellent anaesthetic outcome, either when used alone, or in combination with general anaesthesia. In either setting there is the possibility of prolonged post operative analgesia. When PNB is used as the sole anaesthetic technique, additional advantages may accrue from the avoidance of general anaesthesia complication. However, barriers to the wider use of PNB do exist. Incomplete or unsuccessful blocks remain a particular problem, causing patient discomfort and delays to the operation schedule. Lengthy block, slow block onset may cause late starts. If clinicians are to change their practices and adopt USG, it seems reasonable to examine the validity of these claims with reference to the current published literature for the purposes of this assessment; USG will be compared with the current standard technique of PNS. Aim and objectives: This study is done to with objectives to compare the outcomes of supraclavicular nerve block by using USG and PNS in following aspects, Time to perform the block, Onset time, Duration of block, and Complication such as vascular puncture or persistent neurological Symptoms. Methods: A prospective, randomized, single blinded study was conducted on 100 ASA I or II adult patient undergoing upper limb surgeries under supraclavicular brachial plexus block. Patients were randomly divided into two groups. Patients in Group A (n=50) Group B (n=50). Results: Time to perform block was significantly less in Group B(USG).The onset of sensory and motor block was significantly faster in Group B compared to Group A (p<0.05).NO complicates were notred in group B as compared group A. Conclusion: Ultrasound guided block hastened onset of sensory and motor block, improved postoperative analgesia when compared with PNS guided brachial plexus block, and took less time to perform the block without producing any adverse events.
