A comparative study of scalpel versus electrocautary with reference to seroma after modified radical mastectomy at tertiary health care center

Abstract
Background: Breast cancer is the most common malignancy and the major cause of cancer-related death in female population around the world. Besides breast conversation surgery modified radical mastectomy (MRM) still plays an important role in the operation for breast cancer. This study was conducted to compare two methods of tissue dissection in MRM i.e. harmonic scalpel and electrocautary. Aim and Objective: To study the comparison between Modified Radical Mastectomy performed by scalpel and electrocautary with reference to seroma formation. Methodology: prospective study carried out in 80 patients of breast cancer. Study was done in a tertiary care centre. Modified Radical Mastectomy was done by using harmonic scalpel in one group and by using electrocautary in other group. Boh the groups are compared for intraoperative and postoperative outcome specially seroma formation. Results and Discussion: The mean operation time for harmonic scalpel group (120.5±15.46min) was significantly more than for electrocautary group (90.4±12.07min) (p<0.05). Blood loss during operative procedure and drain volume was significantly lower in the harmonic scalpel group than the electrocautary group. The incidence of seroma formation was less in harmonic scalpel group (10%) than in electrocautery group (12.5%).
