Comprehensive study of blunt abdominal Injuries and its management

Abstract

Background: The abdomen is a Pandora’s box and is commonly involved following blunt trauma. The trauma can be either from road traffic accidents, accidental fall from height, sports injury or violence. Blunt trauma can result from either compression (secondary to a direct blow or against a fixed external object) or from deceleration forces. Physical examination findings are sometimes unreliable. Missed intra-abdominal injuries and concealed haemorrhage are frequent causes of increased morbidity and mortality. Hence CT scanning has been used for increased identification of injuries. In view of increasing number of vehicles, consequent increase in high velocity road traffic accidents, rampant increase in construction work, this study has been chosen to study blunt abdominal trauma, its different modalities of management and treatment outcomes Aim: To evaluate the impact of Blunt injury to the abdomen, study its incidence, distribution and various modes and modalities of investigations and treatment. Material and Methods: This is a prospective study on 59 patients conducted in Government Chengalpattu medical college and hospital, Chengalpattu for a period of one year. Results: Road traffic accidents (76.47%) are the major cause of blunt abdominal injuries involving middle age group males (84.31%). Spleen is the most common organ involved (35.29%). Prompt resuscitation with relevant investigations are significant to decide the organ injury of which CT is the best. About 43% of patients have associated injuries skeletal injury being more commonly associated. Conservative management was carried out in 21.57% of the cases. Mortality and morbidity are high if the time interval between injury and surgery is prolonged hence they must be identified and intervened at the earliest. Conclusion: A thorough and relevant evaluation and quick intervention leads to successful treatment in these patients. Multiple injuries are usually involved in most of the cases rather than an isolated abdominal injury.
