Outcome of minor anorectal surgeries under local anesthesia versus spinal anesthesia at tertiary health care center

Abstract
Background: Anorectal surgeries constitute one of the most frequent surgeries performed by the surgeons. Although usually considered minor surgeries, the associated morbidity of these procedures can be quite debilitating. Aims and Objectives: To study Outcome of Minor Anorectal Surgeries under Local Anesthesia versus Spinal Anesthesia at tertiary health care center. Methodology: This was cross-sectional study carried out in the patients with various Ano-rectal problem like piles, fistula and fissure etc. who requires surgical intervention at the department Surgery at tertiary health care centre during the one year period i.e. January 2017 to January 2018 in the one year period there were 50 patients with the written consent for the Ano-rectal surgery were explained about the procedure of surgery and anesthesia were randomly enrolled to Spinal anesthesia Group, Group (SP) and Local anesthesia Group i.e. Group (L). The statistical analysis was done by Chi-square test and unpaired t-test calculated by SPSS 19 version software. Result: The pain perceived by the patients in both Group was in VAS was (Mean ±SD) was significantly higher at 1st day was 7.82 ± 2.96 and 9.75 ± 3.21 (P<0.0319 t = 2.21; df = 48); 6th day - 3.45 ± 2.12 and 5.67 ± 3.25 (P<0.0319 t = 2.21, df = 48). 1 month - 1.13 ± 1.87 and 3.45 ± 2.13 (p<0.05 t = 2.13, df = 48). Post -operative complains were significantly less in Group L as compared to Group SP Bleeding per rectum-12% and 36% (p<0.05,z =1.98.*), 16% and 28% (p>0.05,z=1.02); Mucus Discharge-8% and 32% (<0.03, z =2.1213*.) Painful Defecation -12% and 40% (p<0.02, z=2.25*.) The Excellent outcome were most common in Group L i.e. 36% as compared to 8%; Good -48 and 32%, Fair -16% and 60% this was statistically significant (p<0.003, X2=11.62, df=2). Conclusion: It can be concluded from our study the patients with local anesthesia were having superior outcome like Excellent outcome, less pain and less complains like Bleeding per rectum, Mucus Discharge, Painful Defecation except Mass per rectum
