Clinico-radiological diagnosis of perforated peptic ulcer: A tertiary care hospital experience

Abstract

Background: The diagnosis of peptic ulcer perforation and secondary peritonitis is mainly established based on the characteristic symptoms. In some cases, doctors may treat ulcers without diagnosing them with specific tests. The classical signs and symptoms for peptic ulceration can raise the suspicion for peptic ulcer and warrant rapid clinical and radiological investigations. Material and Methods: The present study consisted of 50 consecutive cases of perforated peptic ulcers admitted over a period of two years. All the cases were studied with respect to risk factors, clinical features at the time of presentation, radiological investigations and operative findings. Results: A total of 34 (68%) of the patients had history of pain in epigastric region radiating to right hypochondrium and 16 (32%) had diffuse pain. Only 3 cases had doubtful free gas under diaphragm, otherwise 47 cases showed frank free gas under diaphragm.
