A Comparative Study Of Treatment Of Midshaft Clavicle Fracture Between Plate Osteosynthesis And Non-Operative Treatment

Abstract

Background:Clavicle fractures are common fractures of the shoulder girdle occuring more commonly in men especially in the region of middle one-third of clavicle. Methods of treatment include both conservative and surgical modalities. This study was conducted with the aim of comparison between non operative treatment in the form of figure of eight bandage and operative treatment with plate osteosynthesis in displaced mid-shaft clavicle fractures. Materials & Methods:Between 2012-2014, 66 patients with displaced mid-shaft clavicle fractures were enrolled and randomised in two groups of either operative or non-operative management. Operative treatment included open reduction and internal fixation with plates and screws using anterosuperior approach and non operative treatment included sling and figure of eight bandage Results:comparison of patient satisfaction showed 78.8% patients were satisfied with treatment in operative group and 60.6% patients were satisfied in the non operative group. Conclusion:Non union and delayed union are significant risks with non operative treatment although functional results are similar with both forms of treatment. We recommend surgical treatment for these patients but it needs to be individualised for every patient.
