A rare case presentation of full term primary abdominal pregnancy with live fetus

Abstract

Introduction : Abdominal pregnancy is a rare form of ectopic pregnancy with a high rate of maternal and fetal complications. Most of the reported cases are diagnosed in early trimesters, usually after presenting with complications. Sometimes advanced abdominal pregnancy poses a huge diagnostic and management challenge, particularly in low-income countries. A term abdominal pregnancy diagnosed and resulting in a surviving neonate is quite rare.Timely surgical intervention is imperative to avert maternal and fetal complications. Case presentation : A 35 year old female, with H/O 9 and half MOA as G5P4L4 for post-datism with full term, cephalic longitudinal lie. Patient was induced but no progress was seen so patient was taken for LSCS.Directly amniotic sac with fetus was seen in peritoneal cavity. Baby was delivered-2.7kg, male child, Cord was clamped and cut, Placenta was adherent over lower part of large intestine. Part of placenta was adherent to intestine so it was left in situ. Uterus, tubes and ovaries were normal. Post operatively patient was stable and given supportive treatment. patient was discharged with stable vitals and alive neonate. Conclusions : An abdominal pregnancy diagnosed and delivery of a surviving neonate is rare. A high index of suspicion and thorough clinical and ultrasound evaluation are crucial to making an early diagnosis. This is particularly important in areas where advanced imaging technologies are not readily available. Timely surgical intervention is imperative to avert maternal and fetal complications.
