A study of clinical and immunological profile of rheumatoid arthritis in a tertiary care centre in Western India

Abstract
Background: Rheumatoid arthritis () is a chronic inammatory autoimmune disease characterized by joint destruction.is study was conducted to evaluate the clinical, biochemical and immunological prole of and to correlate between severity of clinical features in form of joint counts and level of ESR, CRP , RF, anti CCP. Materials and Methods: Sixty patients above 16 years of age with rheumatoid arthritis diagnosed on the basis of ACR-EULAR 2010 criteria were included in the study. Non-articular and other connective tissue diseases were excluded. Number and paern of joint involvement was noted, and extraarticular features were seen. Inammatory markers (ESR,CRP) were done and titres of RF and anti CCP were evaluated. DAS 28 (ESR) was calculated. Various clinical parameters were correlated with levels of inammatory markers and anti CCP. Results: Mean age of the patients was 38.13 + 12.98 years. 86.66% were females. Mean tender joint count was 18.61+ 6.51 joints and mean swollen joint count was 9.56+ 3.94 joints. Ulnar deviation of the hand, swan neck and Z deformities were the commonest. Rheumatoid nodules and interstitial lung disease (usual interstitial pneumonia) were the commonest extraarticular manifestations. 54.1% patients had a positive CRP and 65% had positive RF. All patients had a positive anti CCP. 8.33% showed erosions on X Ray. Among patients with joint counts > 10, 56.36% had a positive CRP , 52.73% had a positive ESR and 74.55% had high anti CCP. Average DAS 28 was 5.36 (high). ere was a signicant association between titres of anti CCP and DAS 28 , indicating that anti CCP titres were prognostic for disease activity. Conclusion: Most affected were young females with a high joint count.Rheumatoid nodules and ILD were the commonest extraarticular features. Anti CCP was more accurate than RF in predicting the diagnosis of . e average disease activity was high. DAS 28 correlated positively with titres of anti CCP,hence can be used to predict prognosis.
