A study of open versus laprosopiccholecystectommy at tertiary health care centre

Abstract
Background: Prevalence of gallstone ranges from 10 to 20% in India.1 It affects nearly 4.3% of the population. Aims and Objectives: To Study Open versus Laprosopiccholecystectommy at tertiary health care centre. Methodology: This was a hospital based cross-sectional study carried out in the department surgery during the one year period i.e. March 2017 to March2018. one year there were 81 cases selected for study in that 46 allotted to Laparoscopy and 35 to open. The statistical analyses was done by Chi –square test, unpaired t-test calculated by SPSS 19 version software Result: In our study we have seen that The majority of the patients were in the age group of 20-30i.e. 40.00% followed by 10-20 were 24.29%, 30-40were 18.57%, 50-60were 12.86%, >60were 4.29% respectively. The majority of the patients were Male i.e. 67.14% and female were 32.86%The duration of surgery was significantly higher for Laparoscopy as compared to Open (X2=11.43, df=4, p<0.05) The post operative parameters Post-operative pain(VAS- Score )(mean ±SD)-5.45 ± 3.23 and 7.35±4.71(t=3.09,df=79,p<0.05); Post operative hospital stay (mean ±SD)-2.51±1.81and 5.92 ±2.89(t=5.91,df=79,p<0.05); Post-operative duration to return work- 6.76 ± 3.75 and 17.92 ± 7.82(t=9.21,df=79,p<0.001)were significantly more in open surgery as compared to Laproscopic. Overall the complication in Open were significantly more common in Open as compared to Laproscopic ;most of the complications like Vomiting i.e. 22.86 % as compared to6.52%, Abdominal abscess -14.29 %, 2.17% ; Wound infection -11.43%, 2.17%; Ileus -14.29%, 0.00% more in Laparoscopy as compared to Open surgery respectively. (p<0.05). Conclusion: It can be concluded from our study that with the exception of more time is required for laparoscopic cholecystectomy; laporscopy is superior to open surgery with respect to cholcystectomy.
