A study of various perinatal outcomes of patients with pregnancy induced hypertension: A rural tertiary teaching hospital based study

Abstract

Background: Worldwide, 10 % of all pregnancies are complicated by hypertension, with pre-eclampsia and eclampsia being the major causes of maternal and neonatal morbidity and mortality. Aims and Objectives: To study various perinatal outcomes of patients with Pregnancy Induced Hypertension: a rural tertiary teaching hospital based study. Methodology: This was a retrospectivecase control study among the pregnant woman attending the OPD of OBGY at rural tertiary teaching hospital during the two years period i.e. July 2015 to June 2017 was done. The Sample size was calculated by S= 4pq/l2, as per the National Health Portal of India, the prevalence of PIH in India is 10 %, so sample size calculated to be 900 considering allowable error 20%.The statistical analysis was done by SPSS 19 version of software. Result: In our study the prevalence of PIH was 20%, this was in accordance with The prevalence of preeclampsia observed in the study by G. B. Doddamani11 was approximately 10%. The complications found significantly higher in the patients with PIH as compared to Normal ANC patients ; these were HELLP Syndrome (X2=38.92,df=1,p< 0.0001), PPH (X2=12.85,df=1,p< 0.0001) APH (X2=11.25,df=1,p< 0.0001) Cesarean Section (X2=212.1,df=1, p< 0.0001), Intracranial Hemorrhage, X2=11.17,df=1, p< 0.0001). The complications in Fetus found significantly higher in the patients with PIH as compared to Normal ANC patients ; these were Preterm birth (X2=39.12,df=1, p< 0.0001), IUGR (X2=13.75,df=1, p< 0.0001 ), LBW (X2=14.39,df=1,p< 0.0001), Abortion (X2=27.2,df=1, p< 0.0001), IUD (X2=13.82,df=1,p< 0.0001). The complications in Neonate found significantly higher in the patients with PIH as compared to Normal ANC patients ; these were Respiratory distress syndrome (X2=17.41,df=1p< 0.0001), Sepsis (X2=32.5,df=1, p< 0.0001), Intracranial bleed- (X2=19.52,df=1, p< 0.0001). NICU Admission (X2=23.43,df=1,p<0.001), Low APGAR (X2=27.82,df=1, p<0.001 ). Conclusion: It can be concluded from our study that the most common maternal complications found were HELLP Syndrome, PPH, APH, Cesarean Section and the complication common in fetus were Preterm birth, LBW, Abortion, IUD, and Respiratory distress syndrome, Sepsis, Intracranial bleed.
