Abdominopelvic computed tomography in acute abdominal pain: Observational study

Abstract

Background: Acute abdominal pain is a common presenting complaint with a long list of differential diagnosis and variable rates of accuracy with diagnosis based on clinical examination. Of late, Computed Tomography has been increasingly used as an imaging technique for assistance in the evaluation of patients with acute abdominal pain. This observational study was done to assess the role of Computed Tomography in acute abdominal pain patients at our tertiary care centre. Methods: This was an observational study involving all patients with obscure acute abdominal pain attending our institute at Aurangabad from August 2015 to September 2017. Patient evaluation was done by history, clinical examination, ultrasonography, plain abdominal radiograph (X ray abdomen standing) and the patient was sent for computed tomography if there was diagnostic dilemma after aforementioned preliminary investigations. Patients requiring emergency surgery, an urgent CT scan (abdominal trauma), pregnant women and patients not willing for computed tomography or having drug allergy and deranged renal function tests were excluded from the study. CT findings as evaluated by expert radiologist were recorded. After evaluation of CT images, post CT diagnoses were made and treatment planned accordingly. Final diagnosis was made on follow up evaluation and after Histopathological confirmation. Evaluation of role of CT was done by comparing the diagnosis and treatment plan before and after CT results. Results and Conclusions: The study comprised of 103 patients with 65 males and 38 females. There were a higher number of patients in 41-50 years age group. There was a significant change in both the diagnosis and treatment plan after CT evaluation with an increase in cases requiring surgical management. Thus, CT was found to be very helpful in diagnosis and treatment of patients with obscure acute abdominal pain as per our study results.
