Acute otitis media in patients treated in Department of Pediatric Otolaryngology at Medical University of Warsaw in 2014

Abstract

 Introduction. Acute otitis media (AOM) is among the most common bacterial infections in childhood. Thus, it is a common reason for prescribing antibiotics to children. Introduction of polyvalent pneumococcal vaccines decreased an incidence of AOM. Even though resistant strains are still observed. Acute mastoiditis is a rare and severe complication of AOM which requires an urgent hospitalization. Aim. To analyze and assess the etiopathology, dispensed antibiotherapy, indications for surgical management and complications rate for children with AOM. Material and methods. Retrospective analysis of children hospitalized due to AOM. Additionally, telephone survey among parents of the patients was conducted. The data about socioeconomic status and previous pneumococcal vaccination were collected. Results. There were 81 children aged 10 weeks–9.9 years identified with an AOM diagnosis. 31% of patients were admitted as a first episode of AOM. Cefuroxime axetil, amoxicillin with clavulanic acid and clarithromycin were agents prescribed most commonly by primary care physicians. Pneumococcal vaccine was administered in 60% of patients. All of them received 13-valent pneumococcal conjugate vaccine. Parents of 34.7% patients confirmed a potential cigarette smoke exposition. Signs of acute mastoiditis occurred in 10 cases which constituted 12.3% of the cohort. Vaccinated children required surgical management less commonly. Conclusions. Diagnosis and management of AOM can be problematic for primary care physicians. Adherence to treatment guidelines seems to be poor. High use of 2nd generation of cephalosporin and macrolides as a first line agent leads to penicillin unsusceptibility and higher rates of AOM complications what was observed in our study.
