Advances in diagnosis of multiple myeloma and therapeutic recommendations

Abstract

Multiple myeloma is a multistep disease characterized by monoclonal plasma cells proliferation and accumulation.In the past decade dramatic progress in the diagnosis and therapy of multiple myeloma was achieved. Aetiology of disease is unknown. Immunoglobulin heavy chain gene translocations are present in 40-70% of newly diagnosed multiple myeloma patients. Specifically myeloma can be divided into hyperdiploid and non-hyperdiploid subtypes that differ in clinical course of disease. Cytogenetic studies allowed to select a group of high risk patients in which new drugs should be applied. New drugs such as thalidomide, lenalidomide and bortezomib allow to obtain in more than 80% of patients therapeutic response. The integrations of thalidomide, lenalidomide and bortezomib into existing treatment approaches raised several questions regarding the role and place of high dose therapy (HDT) supported by HDT and PBSCT (peripheral stem cell transplantation). In this review arguments supporting the place and role of HDT and PBSCT as a gold standard in patients younger than 70 years raised a question for discussion. Despite unusual progress in the therapy of multiple myeloma none of the available drugs are curative.
