Allergic reacton to long – term Benzathine penicillin injecton for secondary preventon of acute rheumatc fever and recommendatons for skin testng

Abstract
Background Rheumatc Fever (RF) causes 25-40% of all cardio vascular disease in developing countries. Long term benzathine penicillin injecton is being used for secondary prophylaxis of RF / RHD. Although allergic reacton to penicillin is rare skin testng is performed routnely before each and every penicillin injecton delivery in most of the hospitals in Nepal. Objectves Objectves of this study was to evaluate safety of long term benzathine penicillin injecton and establish recommendatons for penicillin skin testng. Methods Data from the registers of Natonal RF/RHD preventon and control programme from 32 hospitals of Nepal were collected and analyzed in a retrospectve study. Results 65 patents (1.4%) among 77300 injectons of benzathine penicillin given to 4712 patents, had allergic reactons. 5 had anaphylaxis, an incidence of 0.1% (0.7/10000 injectons), 60 had minor allergy, an incidence of 1.3%. Conclusions Life-threatening allergic reactons are very rare in patents on long-term intramuscular benzathine penicillin for secondary preventon of RF. With these rare complicatons, regular skin test before each and every benzathine penicillin injecton delivery has no signifcant role. Nevertheless Skin testng is recommended before 1st injecton and patents having different batch number and or brand name.
