An analysis of the maxillary and mandibular alveolar bone to determin the optimal locations for insertion of orthodontic microimplants. Based on a CBCT analysis

Abstract

The primary goal of orthodontic microimplants is to support an orthodontic treatment as anchorage. Surgical procedure is performed in local anaesthesia following an analysis of position of tooth roots. Aim. [b]The aim [/b]of this study was to determine the safest location within the maxilla and mandible for the insertion of orthodontic miniimplants based on measurements of the alveolar bone process using CBCT imaging. [b]Material and methods[/b]. In the present study, 30 tomographic examinations performed using the GENDEX GX CB-500 tomograph at the Department of Oral Surgery, Medical University in Warsaw were used. Measurements of alveolar bone at the interradicular areas in the maxilla and in the mandible as well as on the palate were taken. [b]Results[/b]. The biggest amount of bone in a maxilla was found at the height of 9 mm from the tip of the maxillary alveolar process at the location of molars both in the bucco-palatal plane and in the mesio-distal planes, with an average height of 14 mm and 3.44 mm, respectively. In a mandible, the biggest amount of bone were also obtained at the height of 9 mm from the top of the mandibular alveolar process and, on average they were 12.94 mm in the bucco-lingual plane at the location of molars and 3.75 mm in the mesio-distal plane at the location of premolars. [b]Conclusions[/b]. The safest location for the placement of microimplants in a maxilla are the locations between a second premolar and a first molar, a canine and a first premolar, and between central incisors. In a mandible, the safest locations with the lowest risk are: the space between a first and a second premolar, a first and a second molar and a second premolar and a first molar.
