AN UNUSUAL LARGE LEIOMYOMA OF ROUND LIGAMENT WITH DEGENERATIVE CHANGES IN A HIV 1 POSITIVE CASE

Abstract

A 45-year-old P1 L1 A1, HIV1 seropositive woman, presented with a large abdominal mass. She underwent four surgeries in the past, first one for uterus unification, second one for caesarean section, third one for repair of umbilical hernia and fourth one for intrauterine death with disseminated intravascular coagulation. She received four units of blood six years back during the last surgery. She was later found to be seropositive for HIV1, as she underwent investigations for the abdominal mass. Her husband was seronegative for HIV. Contrast enhanced computed tomography (CECT) abdomen revealed large heterogeneous mass with moderate right hydronephrosis. During laparotomy, the mass was found to be arising from right round ligament and was excised with total abdominal hysterectomy and bilateral salpingo-oophorectomy with partial omentectomy. Histopathology revealed round ligament leiomyoma with hyaline and mucoid degeneration. The mass with degenerated contents measured 40 x 28 cm. and weighed 7.4 kg. The review of literature revealed it to be the largest.
