Analysis of changes in routine blood parameters and thyroid hormone levels in women at different stages of pregnancy and their correlations with TPOAb

Abstract
ObjectiveTo investigate the patterns of changes in routine blood parametersï¼ thyroid hormone levelsï¼ and their correlations with thyroid peroxidase antibodies ï¼TPOAbï¼ among women at different stages of pregnancyï¼ so as to provide a theoretical basis for maternal and child health care and diagnosis and treatment.MethodsA total of 732 pregnant women at different stages of pregnancy who underwent health examinations at the First Maternity and Infant Hospital affiliated to Tongji University from May 2020 to August 2022 were selected as the observation group. The group comprised 245 women in the first trimester ï¼â¤12 weeksï¼ï¼ 247 women in the second trimester ï¼between13 and 27 weeksï¼ and 240 women in the third trimester ï¼â¥28 weeksï¼. Additionallyï¼ 240 non-pregnantï¼ healthy women of child-bearing age who conducted their health checkups in the same hospital were selected as the control group. All the research subjects were required to provide peripheral venous blood samples to measure hemoglobin ï¼Hbï¼ï¼ standard deviation of red blood cell distribution width ï¼RDW-SDï¼ï¼ coefficient of variation of red blood cell distribution width ï¼RDW-CVï¼ï¼ platelet ï¼Pltï¼ countï¼ platelet distribution width ï¼PDWï¼ï¼ as well as thyroid stimulating hormone ï¼TSHï¼ï¼ total thyroxine ï¼TT4ï¼ï¼ total triiodothyronine ï¼TT3ï¼ï¼ free thyroxine ï¼FT4ï¼ï¼ free triiodothyronine ï¼FT3ï¼ï¼ and TPOAb. The results were statistically analyzed.ResultsWith advancing gestational ageï¼ Hb levels were significantly lower in the second and third trimesters than in the first trimester and the control group ï¼F=68.25ï¼ P<0.001ï¼ï¼ while RDW-SD and RDW-CV were significantly higher ï¼F=41.34ï¼ P<0.001ï¼ F=3.64ï¼ P=0.012ï¼. Plt levels throughout pregnancy were significantly lower than that in the control group ï¼F=43.21ï¼ P<0.001ï¼. TSH levels were significantly lower in the first and second trimesters than in the control group ï¼Z=53.49ï¼ P<0.001ï¼ï¼ but gradually increased with gestational age. TT3 and TT4 levels were significantly higher than those in the control group throughout pregnancy ï¼Fï¼148.25ï¼ P<0.001ï¼ Fï¼210.83ï¼ P<0.001ï¼ï¼ while FT3 and FT4 levels were significantly lower in the second and third trimesters than in the first trimester and the control group ï¼F=42.95ï¼ P<0.001ï¼ F=101.73ï¼ P<0.001ï¼. The abnormal rate of TPOAb was significantly higher than that in the control group throughout pregnancy ï¼Ï2ï¼25.61ï¼ P<0.001ï¼. Among pregnant womenï¼ those with TPOAb positivity had significantly higher TSH levels and RDW-CV than those with TPOAb negativity ï¼Zï¼5.70ï¼ P<0.001ï¼ tï¼2.39ï¼ Pï¼0.018ï¼.ConclusionThe levels of Hbï¼ Pltï¼ and thyroid hormones in pregnant women are closely related to gestational age. With increasing gestational ageï¼ the abnormal rate of TPOAb decreasesï¼ but the TSH levels and RDW-CV of TPOAb positive pregnant women are higherï¼ requiring clinical attention and screening to improve maternal and child health.
