Angle Class II subdivision malocclusions – review of treatment modalities

Abstract
Unilateral class II subdivision malocclusion is an asymmetric variation of Class II malocclusion where a correct relationship in Class I is present on one side only. Taking into account major etiological factors of unilateral Class II, we can distinguish two types of the disorder. Type 1, in which the midline of the upper dental arch coincides with the facial midline and asymmetry appears in the lower dental arch. Type 2, in which the midline of the lower dental arch coincides with the facial midline, asymmetry of the upper dental arch is characterized by a mesial position of the first maxillary molar on the side of the defect. The treatment of this malocclusion poses a great challenge for clinicians due to difficulty diagnosing the cause of this disorder. [b]The aim[/b] of this study was to investigate the incidence of Class II subdivision malocclusion and to analyze the treatment methods applied for asymmetric class II. [b]Material and method.[/b] Records were selected of 439 patients with Angle Class II malocclusions who were treated in BIOORTO Orthodontic Clinic in Wroclaw between 2004 and 2012. There were 17% of patients with Class II subdivision in the study group. [b]Results.[/b] The most commonly used appliances / therapeutic methods in our study were extractions of premolars (symmetric and asymmetric), asymmetric elastics, a modified Herbst appliance, and space restoration for the missing or impacted tooth. [b]Conclusion[/b]. The study demonstrated that type 1 of unilateral Class II (asymmetry in the lower dental arch) occurs twice as often as type 2 (asymmetry in the upper dental arch).
