Application of cerebral electrical impedance tomography in the diagnosis of stroke after Stanford type A aortic dissection operation

Abstract

Objective To investigate the value of cerebral electrical impedance tomography (EIT) in the diagnosis of stroke after Stanford type A aortic dissection ( TAAD) operation. Methods A retrospective analysis was performed on 59 patients who underwent cerebral CT examination and EIT monitoring after TAAD operation in the Department of Cardiovascular Surgery, Xijing Hospital, Air Force Medical University from December 2020 to October 2021. The value of EIT monitoring in the diagnosis of stroke was determined based on the results of cerebral CT. Results Among the 59 patients after TAAD operation, 24 (40. 68% ) had cerebral CT imaging changes, and 35 (59. 32% ) had no significant imaging changes. Comparative analysis showed that the sensitivity, specificity, and accuracy of EIT monitoring were 75. 00% , 54. 29% , and 57. 62% , respectively. Among the 15 new cases of cerebral infarction and cerebral hemorrhage, 13 cases of suspicious lesions were monitored by EIT, and the monitoring accuracy was 86. 67% . Among the 59 patients, 52 patients recovered and were discharged from hospital, with an overall recovery rate of 88. 14% . There was no statistical significance between the overall recovery rate and the recovery rate of cerebral CT positive patients (75. 00% ) and negative patients (97. 14% ), the recovery rate of EIT positive patients (86. 49% ) and negative patients (90. 91% ), the recovery rate of true-positive patients (72. 22% ) and true-negative patients (93. 75% ), and the recovery rate of false-positive patients (100% ) and false-negative patients (83. 33% ). Conclusion As a new method of nervous system monitoring, EIT has certain clinical application value in the diagnosis of stroke after TAAD operation.
