Assessment of indications for extraction of permanent teeth and of their number during orthodontic treatment

Abstract

Orthodontic treatment planning is often connected with the necessity of taking a decision to extract permanent teeth. Some of the extractions are performed pursuant to strictly orthodontic indications (crowding, compensating extractions) and it is a method of treatment of dentalocclusal defects, which has been applied for a long time. The remaining types of extraction are conducted due to, e.g.: caries complications, trauma, or improper structure, number or location of teeth. This topic is subject to constant verification as a result of the development of science and changing therapeutic possibilities. [b]The aim of the stud[/b]y was to define the indications for extraction, the number of extracted teeth as well as the number of patients for whom the decision of permanent tooth extraction was taken while under orthodontic treatment. [b]Material and methods[/b]. An analysis was made of the medical documentation of patients who had been treated in the Orthodontic Clinic of the University Dental Centre (UDC)of the Medical University of Gdansk in the years 2009-2010. A total of 470 patients aged between 7 and 36 was examined, out of whom 239 were female patients and 231 were male patients. [b]Results.[/b] Orthodontic treatment with extraction of permanent teeth was conducted in 31 (6.6%) people, 17 of whom were girls, and 14 were boys. In19 (61.29%) patients extractions took place during the treatment of crowding. Other indications included 6 compensating extractions (19.35%), 4 caused by a trauma (12.90%), one by caries complications (3.23%) and one by an abnormal number of teeth (3.23%). [b]Conclusions[/b]. In diagnostically justified cases of orthodontic treatment it is necessary to take a decision to extract permanent teeth . This method should be taken into account when planning treatment; however, the decision to perform an extraction has to be taken with caution, since the effects are irreversible. Recently, due to the development of new treatment techniques, a decline has been observed in the number of permanent tooth extractions performed in patients under orthodontic treatment.
