اثرات سه دوز مختلف سيس¬آتراكوريوم بر زمان مناسب برای لوله¬گذاری داخل تراشه و تغييرات هموديناميك طی بیهوشی

Abstract
زمینه و هدف: سیس¬آتراکوریوم به منظور تسهیل لوله¬گذاری داخل تراشه مورد استفاده قرار می¬گیرد، اما دوزهای معمول آن شروع اثر کندی را به¬دنبال دارد. هدف از این مطالعه مقایسه اثرات سه دوز مختلف سیس¬آتراکوریوم(15/0 و 20/0 و 25/0 mg/kg) برتغییرات همودینامیک و زمان مناسب برای لوله-گذاری داخل تراشه می¬باشد. مواد و روش¬کار: این مطالعه کارآزمایی بالینی بر روی 123 بیمار 20 تا 40 ساله با وضعیت فیزیکی در کلاس 1 یا 2، به¬طور تصادفی از یکی از سه دوز سیس آتراکوریوم 15/0، 20/0 و 25/0 mg/kg برای تسهیل لوله¬گذاری داخل تراشه استفاده شد. پس از القای بیهوشی فشارخون شریانی و تعداد ضربان قلب قبل از القای بیهوشی و پس از آن هر یک دقیقه تا 5 دقیقه بعد از القای بیهوشی و سپس، هر یک دقیقه تا 5 دقیقه پس از لوله¬گذاری داخل تراشه اندازه¬گیری و ثبت گردید. زمان مناسب برای لوله¬گذاری داخل تراشه (صفر شدن train of four و Post Tetanic Coun) و بازگشت train of four پس از پایان تزریق سیس¬آتراکوریوم ثبت گردید. یافته¬ها: پس از القای بیهوشی، بیماران در هر سه گروه دچار افت قابل توجه فشارخون شريانی شدند) 001/0p<). ضربان قلب نیز کاهش مختصری نشان داد ) 05/0p>). بیش¬ترین تغییرات به¬ترتیب در گروه 20/0، 25/0 و 15/0 بود. اما از نظر بالینی قابل اهمیت نبوده و نیاز به مداخله دارویی نداشتند. سریع¬ترین زمان مناسب برای لوله¬گذاری به¬ترتیب در گروه 25/0، 20/0 و 15/0 مشاهده شد (001/0p<). زمان بازگشت train of four در گروه 15/0، 20/0 و 25/0 به¬ترتیب 25/324±20/2696 ثانیه، 2/626±9/3378 ثانیه و0/253±2/3422 ثانیه بود (001/0p<). نتیجه¬گیری: استفاده از دوز 20/0 و 25/0 mg/kg از داروی سیس¬آتراکوریوم، به¬جای دوز 15/0، باعث تسریع در ایجاد شرایط مناسب لوله¬گذاری تراشه می-شود و اگر چه با تغييرات همودینامیک قابل ملاحظه¬ای نیز همراه است اما از نظر بالینی قابل اهمیت نبوده و نیازمند مداخله نیستند.]م ت ع پ ز، 1390; 13(7):17-13 [ کليدواژه¬ها: سیس¬آتراکوریوم، لوله¬گذاری داخل تراشه، همودینامیک، بلوک عصبی عضلانی
