Audit of repeat fine needle aspiration in cytopathology laboratory of tertiary health center

Abstract

Background : Fine needle aspiration cytology offers clear advantages to patients and doctors. The technique is minimally invasive, produces a speedy result and is relatively inexpensive. Its accuracy in many situations, when applied by experienced and well-trained practitioners, can approach that of histopathology in providing an unequivocal diagnosis. The success of fine needle aspiration cytology (FNAC) depends to a high degree on perfecting the technique of sampling and preparation of samples. Methods : It's a retrospective study conducted in the cytopathology laboratory of Pathology department at Government Medical College and Sir T Hospital, Bhavnagar, Gujarat over 2 years from year 2015 and 2016. All patients who attended the cytopathology laboratory for the FNAC procedure and USG guided FNAC were included in the study. We had noted the reasons for repeats and proportions of repeated aspirates. Results : Total 2566 patients of Sir T Hospital, Bhavnagar were subjected to FNAC during two year duration. Out of this, 673 FNAC samples aspirated from the lymph nodes, 137 from the salivary gland, 582 from breast swellings, 302 from the thyroid lesions, 254 were CT/USG guided aspirate from lung and 618 were from miscellaneous lesions. Total 783 case (30.5%) were subjected to repeat aspiration due to various reasons like in first aspirate obtained only blood 321 case (41%), inadequate cellularity 308 case (39.4%) and descriptive but non-diagnostic aspiration 154 case (19.6%). Thyroid (42.3%) and breast (34.1%) were have higher proportion of repeat FNAC while CT/USG guided lung FNAC had lower proportion(15.7%) of repeat FNAC to make a diagnosis. Conclusion : In our study of audit of repeat FNAC, repeat aspiration due to reason in order of occurrence were aspirate obtained only blood, inadequate cellularity and descriptive but non-diagnostic aspiration. Malignant lesion had lower repeat FNAC rate than others
