Bacterial diagnosis of pneumopathies: Contribution of the cytobacteriological examination of the sputum (CBES) in a pulmonological environment Black Africa

Abstract
Bacterial pneumopathies are common reasons for consultation in pneumology. The etiological diagnosis of these pneumonitis is expensive. CBES (Cytobacteriological examination of sputum) is a non-invasive and inexpensive examination. It allows the precise identification of the bacterial ecology. It is an important tool for the rehabilitation of antibiotic therapy. The purpose of our work was to determine the interest of CBES in the diagnosis of bacterial pneumonia. This is a prospective and analytical study from November 2013 to November 2014. The frequency of bacterial pneumopathies in the department was 15.5% (271/1745). CBES isolated a seed in 62.7% (170/271). The predominance was male with a sex ratio of 1.27. The average age was 34 years old with the extremes 12 and 84 years old. The 26-45 age group was the most represented (57%). A history of pulmonary tuberculosis was found in 5%. HIV serology was positive in 7 patients, ie 4%. Smoking accounted for 28%. The bacilloscopy was negative in all patients. The most common germ found was Klebsiela pneumoniae (46%). The most used antibiotic was 3rd generation of Cephalosporines. The average duration of treatment was 16 days with extremes ranging from 10 to 30 days. The evolution was favorable in all patients. Conclusion. CBES is a key and non-invasive test in the diagnosis of bacterial pneumonia, especially in low-income countries.
