Blood glucose level and insulin resistance in patients with type 2 diabetic mellitus, diabetic retinopathy and obesity
Journal Title: Міжнародний ендокринологічний журнал - Year 2017, Vol 13, Issue 3
Abstract
Background. Diabetic retinopathy is the main cause of visual disorders in the world. The purpose of our work was to study the level of glycaemia and insulin resistance in patients with type 2 diabetes mellitus (DM), diabetic retinopathy and obesity. Materials and methods. One-hundred and seven diabetic patients (234 eyes) observed were divided in two groups. Results. The levels of blood glucose and insulin resistance were studied at different stages of diabetic retinopathy (DR) in patients with type 2 DM. Using ANOVA it was shown that all patients with DR, regardless of study group, had chronic hyperglycemia with decompensated T2DM. The carbohydrate metabolism was found to begin improving only at the 3rd stage of DR, however it occurs on a background of hypoglycemia. Conclusions. The greatest proportion of patients with adverse indices of insulin resistance by HOMA-IR index (87.5 ± 11.6%) and with reduced sensitivity to insulin by QUICKI index (62.5 ± 17.1 %) had proliferative DR stage compared to the comparative and control groups (obese patients without T2DM): 55.0 ± 11.1%; p < 0.001, and 25.0 ± 9.68 %; p < 0.001, respectively.
