Cervical Epidural Abscess Mimicking Subarachnoid Haemorrhage: An Atypical Presentation

Abstract

Delayed diagnosis in a spinal epidural abscess is related to poor outcome. This case highlights an atypical presentation of a spinal epidural abscess. Early diagnosis leads to early therapy and better outcome. We present the case of a man who had a good clinical history and CSF findings to support a diagnosis of subarachnoid haemorrhage. Following a negative cerebral angiogram, a revised diagnosis of �spinal� subarachnoid haemorrhage was made. Further imaging however revealed an epidural abscess. We use this to discuss the investigation of suspected �spinal� subarachnoid haemorrhage. We also discuss the presentation of an epidural abscess and highlight that the clinical presentation may be atypical.
