Change of Capillaroscopy Data in Patients with Type 1 Diabetes Mellitus in Case of Chronic Complications

Abstract
The aim of this work was to study the relationship of capillaroscopy indicators of the upper extremities vessels with the severity of nephropathy, retinopathy, clinical manifestations of angiopathy of lower extremities in patients with diabetes mellitus type 1. Methods of the Study. We examined 53 patients with type 1 diabetes mellitus: 30 females and 23 males. The investigated persons were divided into 3 groups. The computer capillaroscopy was performed to all patients. Results of the Study. The greatest changes of microvasculature were observed in patients of group III, who had marked microangiopathy stages. Defeat of microvasculature of the upper extremities progressed with the development of the underlying disease and manifested by shortened length of capillaries, decreased density of the capillary network, increased diameter of the capillary transition departments. A density of the capillary network is most clearly characterized the state of blood flow, and its decrease indicated profound violations of microcirculation in the periphery. Decrease in the density of the capillary network had a direct correlation with glomerular filtration rate (ρ = 0.601) and inverse correlation with the presence of protein in the urine (ρ = –0.644). In patients with diabetes mellitus type 1 changes in the structure of the capillaries of the nail bed of the upper extremities progressed synchronously to stages of retinopathy and clinical manifestations of angiopathy of lower extremities.
