Characteristics of HIV-infected persons and their spouses who became HIV-positive in HIV discordant couples in Dehong Prefectureï¼ Yunnan Province

Abstract
ObjectiveTo determine the characteristicsï¼ viral load and immunological status of HIV-infected persons and their spouses who became HIV-positiveï¼ and the reasons for HIV seroconversion in 55 HIV discordant couples in Dehong Dai and Jingpo Autonomous Prefecture ï¼Dehong Prefectureï¼ï¼ Yunan Province.MethodsData on the 55 couples meeting the criteria of having a previously positive spouse were retrieved from the AIDS Integrated Prevention and Control Data Information System of the China Disease Control and Prevention Information System during 2015-2021. General socio-demographic informationï¼ age at diagnosisï¼ exposure historyï¼ CD4+T lymphocyte countï¼ and antiviral treatment were collected. Descriptive analysis and chi-square test were used to compare the distribution of pre-HIV-positive spouses and their HIV seroconverted spouses.ResultsA total of 55 spouses from HIV discordant couples had HIV seroconversion. Of themï¼ 72.7% ï¼40/55ï¼ of pre-HIV-positive spouses were husbands. The most recent CD4+T lymphocyte count in the pre-HIV-positive spouses was ï¼328.31± 246.27ï¼ cells·μL-1 at the time of diagnosis of their seroconverted spousesï¼ of which 36.3% ï¼20/55ï¼ had a CD4+T lymphocyte count of less than 200 cells·μL-1. Furthermoreï¼ of those pre-HIV-positive spouses with low CD4+T lymphocyte countï¼ 45.0% ï¼9/20ï¼ had an undetectable viral loadï¼ 15.0% ï¼3/20ï¼ <400 copies·mL-1ï¼ and 25.0%ï¼5/20ï¼ â¥400 copies·mL-1. Additionallyï¼ 16.4% ï¼9/55ï¼ of the pre-HIV-positive spouses did not have a viral load test. The main reasons for HIV seroconversion among HIV-negative spouses in the discordant couples were poor condom useï¼ poor compliance with antiviral therapyï¼ and treatment discontinuation.ConclusionThe follow-up management of HIV discordant couples should be strengthened in Dehong Prefectureï¼ especially the monitoring of viral load levels and immunological status of pre-HIV-positive spousesï¼ to improve their compliance with antiviral therapy and reduce treatment discontinuationï¼ which would effectively prevent and control HIV transmission between spouses.
