Chest trauma by zebu encornment in Antananarivo Madagascar

Abstract
Introduction. Zebu horn is one of the few etiologies of thoracic trauma in the world. In Madagascar, it happens mostly in rural areas. This trauma is serious because of its very high degree of sepsis. Thus, the objective of our study is to describe the epidemiological, clinical, therapeutic and prognostic aspects of thoracic trauma by zebu encornment seen at CHU-JRA Antananarivo - Madagascar. Method. This is a descriptive and retrospective study of hospital records for thoracic trauma by zebu encornment in the Department of Thoracic Surgery and Cardiovascular Surgery of CHU-JRA Antananarivo. This study was conducted over a 12-year period, from January 2002 to December 2014. Results. We gathered 78 patients and the median age was 38 years with extremes of 4 and 74 years. The majority of these patients were thoracic traumas with parietal wounds of the thorax without pleural disruption in 48.71% of cases (n=38) and the thoracic lesions encountered were mainly represented by rib fractures in 78.20% of cases (n=61). Treatment was medico-surgical in 87.18% of cases (n=68) and treatment progression was favorable in 73.03% of cases (n=57). Conclusion. Thoracic trauma by zebu encornment remains a problem in Madagascar, as well as in other black African countries, India and bullfighting countries in Europe. As it is the preserve of the rural populations, the care is often delayed and inadequate, favoring various septic complications.
