Chronic peptic ulcer disease – a psychosomatic disorder

Abstract

Introduction: The concept of ‘psychosomatics’ was introduced in the first half of the 20[sup] th[/sup] century. According to the American Psychiatric Association (DMS-IV classification), psychosomatic disorders are the effects of psychological factors precisely connected with the beginning or exacerbation of an illness. The main causes of chronic peptic ulcer disease are Helicobacter pylori infection and non-steroid antiinflamatory drugs.Objective: Analysis of the role of emotional factors, stress and type of personality in the pathogenesis of chronic peptic ulcer disease. The major role of psychological factors should not be ignored. One stressful situation cannot cause a peptic ulcer. Through the years, the human organism has gained many defence mechanisms (gastric prostaglandins, regular blood flow in gastrointestinal vessels) protecting against stress. Nowadays, damage to this barrier is more common and the amount of harm is still increasing. About 30-60% of gastric and duodenal ulcers are said to be the result of stress. Chronic peptic ulcer disease is more common among people with type D personality, characterized by a social dysfunction and tendencies for hiding emotions. This is the reason why psychotherapy should be performed as part of the treatment. Other mental disturbances causing progression of the ulcer are anxiety and depression. Due to the co-existence of a somatic and a mental background to the illness, psychological support should be provided. During meetings with the psychotherapist, the patients can learn how to deal with the stress of everyday life.Key words: stress, cortizol, peptic ulcer
