Clinical symptomatology of uterine leiomyoma in relation with size

Abstract

Background: Uterine leiomyomas are the commonest benign uterine tumors. They can be single or multiple and can vary in size, location, and perfusion. Size and location are the main factors that determine if a fibroid leads to symptoms and problem. The symptomatology of uterine fibroid continues to be variable, it is believed that symptoms depend upon size, site and number of fibroid tumor. The present study evaluated the correlation of size of the uterine leiomyoma with its clinical signs and symptoms. Material and Methods: In this study a total of 173 women diagnosed clinically and confirmed with radio-imaging and histopathology examinations as uterine leiomyomas above the age of 21 years were included. Detailed clinical history, physical examination findings including bimanual pelvic examination and investigations were recorded. Transabdominal and transvaginal sonography was done to determine the size and location of the leiomyoma. Results: Leiomyoma were most commonly seen in the women of child bearing age, most commonly occurring in the 3rd decade. The mean age was 39.91 years. They were more common in 2nd and 3rd para women. Symptoms or menstrual disturbance are commonly observed in 159 (91.9%) of total numbers of fibroids and these are common in fibroids 89 (51.6%) which has size between 5-10 cm. Dysmenorrhea was observed in total 55 (32.1%) of fibroids, most commonly in 27 (49.1%) of fibroid which size between 5-10 cm. Discussion: Leiomyoma are usually found in reproductive age group. The incidence of leiomyoma is highest in the 3rd decade. The symptoms of uterine leiomyoma such as menstrual disturbances and dysmenorrhoea in present study are correlate well with their size.
