Comparative prospective study of clinical outcomes in laparoscopic appendicectomy versus open appendicectomy conducted at tertiary level hospital, Vadodara

Abstract

BACKGROUND : Appendicitis is most common intra abdominal inflammatory condition requiring emergency surgery. Despite advances in the diagnostic modalities, the diagnosis is still doubtful in 30 - 40% of cases. Appendicectomy by Mc Burney’s incision remained the gold standard until 1983. Laparoscopic appendicectomy has gained acceptance as a diagnostic and treatment method for acute appendicitis. Many studies have different opinions about these two techniques. So this study is carried out with objectives to compare these two methods of appendicectomy in terms of operative time, post operative pain and hospit al stay. MATERIALS AND METHODS: All patients operated for emergency and interval appendicectomy during August 2011 to October 2012 at Department of surgery, SSG Hospital, Vadodara were considered in the study after taking written informed consent. All these patients were thoroughly investigated pre - operatively. Patients were randomized by closed envelope method. Time taken for surgery, post operative pain, was noted along with hospital stay. RESULTS : Out of 183 patients, 66(37%) patients underwent lapar oscopic appendicectomy and 117 (63%) patients underwent open appendicectomy. The mean duration of surgery in open appendicectomy group was 35.57mins as compared to 49.09mins in laparoscopic group which was highly significant. The post operative pain (VAS s core) was less in laparoscopic group for day 1 (mean 5.57) while in open group (mean 6.14) and for day 2 mean noted 2.0 and 4.65 respectively which was highly significant. Length of hospital stay was significantly shorter in laparoscopic group (2.12 days) compared to open group (5.14days). CONCLUSION : Laparoscopic appendicectomy has advantage of less postoperative pain and shorter hospital stay at cost of slightly increase duration of surgery co mpare to open appendicectomy
