Comparative prospective study of laparoscopic TEP repair versus laparoscopic TAPP repair for inguinal hernioplasty conducted at tertiary level hospital, Vadodara

Abstract

BACKGROUND: Inguinal hernias are most common conditions referred to surgeons all over the world and over half million hernia repairs performed annually. The lifetime risk for men is 27% and for women 3%. Laparoscopic inguinal hernia repair done either by TAPP approach or by TEP approach. Selection of patients for either TEP or TAPP repair difficult due to steep learning curve and lack of documentation of results. Our study was carried out with objectives to compare these two methods of laparoscopic inguinal hernioplast y in terms of duration of operation, complication rate and duration of hospital stay. MATERIALS AND METHODS: Patients who underwent laparoscopic inguinal hernioplasty from December 2011 to October 2013 at SSG hospital were considered in study. All patients were thoroughly investigated pre - operatively. Patients were randomized by sealed cover method. Operative time, duration of hospital stay and complication rate of each approach was noted. RESULTS : Out of 60 patients, 30 patients underwent laparoscopic TEP repair and 30 patients underwent laparoscopic TAPP repair for inguinal hernioplasty. Indirect inguinal hernia was the most common hernia in both the groups. The mean operative time was significantl y higher in TAPP group ( 87±13.6mins) compared to TEP group ( 72±10.4mins). The mean duration of post operative hospital stay was (2.8±1.3) days in TEP group while it was (2.76±1.0) days in TAPP group. The complication rate slightly higher in TEP group (26.7 %) compare to TAPP group (20%). CONCLUSION : Laparoscopic TAPP repair is safe and feasible for inguinal hernioplasty at the cost of increased operative time.
