COMPARATIVE STUDY BETWEEN CONTINUOUS SUTURE AND INTERRUPTED SUTURE IN LAPAROTOMY WOUND REPAIR

Abstract

BACKGROUND There is always a concern amongst the surgeons about abdominal wound complications after continuous or interrupted suturing, so our study is hereby intended to compare the outcome of continuous and interrupted suture in laparotomy wound repair. Aims & Objectives- To measure the risk of abdominal wound dehiscence, wound infections & gaping, seroma and incisional hernia for interrupted and continuous suture technique in laparotomy and to calculate the relative risk of burst abdomen between continuous and interrupted method. MATERIALS AND METHODS Total of 100 patients undergoing midline laparotomy where sample size was taken as per the convenience and admission rate for abdominal surgery at M.Y. Hospital were included in 2 groups, 50 in each group divided alternatively in both groups, in group A (odd number) in which continuous suture was applied & in group B (even number) in which interrupted suture (figure of 8) was applied. Monofilament polypropylene No.1 suture was used in both groups A & B. RESULTS In our study, 15 patients had wound dehiscence in group A in which continuous suture was applied and 3 in group B in which interrupted suture was applied which was significant with P value of 0.002 (<0.05) with Chi value of 8.19. Relative risk of 0.572. Fifteen patients had Wound Gaping in group A and 4 in group B which was significant with P value of 0.005 (<0.05) with chi value of 6.498. Seventeen patients had wound infection in group A, while only 5 had this complication in group B, with P value of 0.004 (<0.05), chi = 7.05 making it a significant difference. Seventeen patients had seroma formation in group A, while 6 had this complication in group B which was significant with P value of 0.009 (<0.05), chi = 5.64. CONCLUSION Interrupted closure of midline laparotomy wound technique is better than continuous closure technique in terms of less incidence of postoperative complications like seroma formation, wound infection, wound gaping, wound dehiscence.
