Comparative Study of Chemical Cauterization of Inferior Turbinate with Inferior Turbinectomy in Allergic Rhinitis

Abstract

Background and objectives: Mitigation of the prominent symptoms of Allergic rhinitis (AR) such as rhinorrhea and nasal obstruction may not always be achieved by conservative, non-surgical measures alone. Hence, this study was conducted to compare the efficacy of chemical cauterization of the inferior turbinate and inferior turbinectomy in the management of AR, in terms of symptom control, post-operative discomfort and complications. Materials and methods: This was a comparative, randomized controlled study with 100 patients with AR whose symptoms were not satisfactorily controlled with standard medical treatment. They were randomly allocated into two groups, with 50 patients in each. Patients in Group 1 underwent chemical cauterization of inferior turbinates with 15% Silver nitrate and those in Group 2 underwent inferior turbinectomy. Symptoms at presentation and at three weeks, three and nine months after procedure were scored on a 0-10 numerical rating scale. The improvements in the symptom scores between the groups were compared. Results: Both groups showed significant improvement of symptoms through nine months of follow-up (p-value < 0.0001, ANOVA). Inferior turbinectomy and chemical cauterization of inferior turbinates were similar in controlling symptoms. Complications were noted only among patients in group-2, while transient post-operative discomfort was more among patients in group-1.Conclusion: Chemical cauterization of inferior turbinates can be a less traumatic surgical procedure for controlling symptoms of allergic rhinitis with an efficacy comparable to inferior turbinectomy. It obviates the need for hospitalization and is associated with lesser incidence of complications.
