Comparative study of Prolene hernia system versus trans abdominal pre-peritoneal mesh repair (TAPP) in primary and recurrent inguinal hernias

Abstract
Background: Worldwide, inguinal hernia repair is one of the most common surgeries, being performed in more than 20 million people annually. The lifetime occurrence of groin hernia - viscera or adipose tissue protrusions through the inguinal or femoral canal - is 27-43% in men and 3-6% in women. Inguinal hernias are almost always symptomatic, and the only cure is surgery. Material and Method: 1. To compare short term and mid-term outcomes of Prolene Hernia System (PHS) with Laparoscopic hernia repair (TAPP) in primary and recurrent inguinal hernia. 2. To evaluate efficacy and benefits of TAPP in inguinal hernia repair. A Prospective Comparative study was conducted at Department of General Surgery and Surgical Gastroenterology, Apollo Hospitals, Jubilee hills, Hyderabad, on 50 subjects of inguinal hernia. All the subjects were interviewed, examined and investigated according to the proforma that was pre-designed and pre-tested between two methods of hernia repair with anterior approach and posterior approach i.e. PHS and TAPP tension free hernia repair. Observation: In this study right sided hernia was more common than left side in both groups (75% Vs 25%). Direct hernias 76% were more compared to indirect hernias 24%. In our study bilateral hernia is common than left side hernia. Majority of the subjects (40.9%) were in the age group of 50-59 years. 40%6/15(PHS 6 + TAPP 9) data taken from 50-59 range of age belongs to PHS group and 60% 9/15 (PHS 6+ TAPP 9) data taken from 50-59 range of age belongs to TAPP group >60% (61.36) subjects were > 50 years and < 70 years of age. Conclusion: TAPP was associated with less post operative pain, a shorter period of sick leave and a faster recovery, compared with PHS repair. TAPP hernia repair may not be more expensive than PHS repair (mesh is costly than routine mesh) in terms of direct hospital costs or where a difference exists, this is relatively small. Societal costs due quicker recovery and return to employment show clear advantages for the TAPP repair.
