Comparative study of Sclerotherapy, Banding and glue injection for Oesophageal varices

Abstract

Background: Cirrhosis is a common cause of portal hypertension and esophageal varices (EV) are present in 30% to 40% of patients with compensated disease (no ascites, encephalopathy or severe jaundice) and up to 60% with decompensated disease. Aims and Objectives: To study Sclerotherapy, Banding and glue injection for Oesophageal varices Methodology: This was a Cross-sectional study carried out in the Study was conducted Surgical Gastroenterology Department of Medical College Hospital, between 2013 to 2014. All the patients which were referred to Gastroenterology department for endoscopy of Esophageal varices and management within one year period i.e. January 2013 to January 2014 were retrieved into study. Within one year total 100 patients were included. These patients were divided into Group I, Group II, Group III as per the consent i.e. - Group I- cyanoacrylate glue. Group II-Sclero therapy using absolute alcohol. Group III- rubber O bands ligation. The statistical analysis done by Chi-square test, un-paired t-test. P<0.05 was considered as significant at 95% Confidence Interval. By SPSS 19 version software. Results: In our study we have observed that The majority of the patients were in the age group of 50-60 i.e. 35% followed by 60-70-25%; 70-80-22%; >80 were 15%; 40-50 were 5%; 30-40 were 2%. The majority of the patients were Male i.e. 75 % and Females were 25%. Variceal obliteration was respectively in Group I was 29% and Group II was 13% and Group III was 14% was statistically significant (P<0.05*, Group I Vs Group II, Group I Vs Group III) and No. of Sessions (mean ±SD) was 4.5 ± 2.1, 10.2 ± 3.1 and 3.9 ± 2.7 also statistically significant (P<0.05*, Group I Vs Group II, Group III Vs Group II). The complications in three different treatment methods were comparable to each other as P>0.05 the most common complication were -Ulceration in 17% followed by Rebleeding in 15%,Dysphagia in 10% , Pyrexia in 9% , Pain in 8%. Conclusion : It can be concluded from our study that as far as complication are concerned all three i.e. cyanoacrylate glue , Sclero therapy, rubber O bands ligation but cyanoacrylate glue found to be superior with respect to Variceal obliteration and less No. of Sessions required.
