Comparative study on the effect of addition of potassium chloride to bupivacaine on the onset time and duration of inter scalene block

Abstract

Background: The interscalene block is a more established technique and has been used routinely for shoulder surgery. It is also more associated with well known, but short-lived, side effects than the supraclavicular technique. Aims and Objectives: to Study the Effect Of Addition Of Potassium Chloride To Bupivacaine On The Onset Time And Duration Of Inter Scalene Block. Methodology: The present study was carried out at Rajah Muthiah Medical College Hospital in a double blind-manner. Sixty patients of age group between 20-70 years of either sex of ASA grade I and II, were selected for the study. Each patient was randomly assigned to one of the two groups (30 patients each), group I or Group II. Group I (Potassium group) received 30 ml of 0.375% bupivacaine with 0.2 mmol of potassium chloride (prepared by adding 0.1ml of potassium chloride and 10ml distilled water to 20 ml of 0.5% bupivacaine). Group, II (Plain bupivacaine group) received 30ml of 0.375% bupivacaine only. These injections were given by interscalene approach. Statistical comparison was performed by student-'t' test and CHI-SQUARE test. Statistical analysis done by SPSS 19 version software. Result: The quality of sensory blockade was better in group I and the value was statistically significant when compared with group II. (X2 (1) =7.34, p=0.028). The quality of Motor blockade was better in group I and the value was statistically significant when compared with group II (X2 (1) =8.42, p=0.028). The number of adjutants used in group I were significantly less when compared to group II. The p value was <0.05 which is significant. Conclusion: It can be concluded that addition of potassium chloride to bupivacaine for brachial plexus block by interscalene route significantly decreases onset time, increases depth of blockade and prolongs the duration.
