Comparison of the effect of different timing of premedication with fentanyl on the hemodynamic stress response to laryngoscopy

Abstract
Background: One of the main challenges an anaesthesiologist faces in the conduct of general anaesthesia is obtundation of the hemodynamic stress response to laryngoscopy and intubation. Various pharmacological interventions have been used to this end and the most popular agent among them is the use of short acting opiods such as fentanyl. In this study we show how the timing of this premedicant is as important as its dosage. Methods: In our study 90 patients undergoing elective surgeries under general anaesthesia were divided into three groups to be premedicated with fentanyl at 3,5,7 minutes respectively and anaesthesia was induced and the heart rate systolic, diastolic and mean arterial blood pressure were recorded at baseline, induction, intubation and at 1,3,5,10 minutes after intubation and compared. Results: in our study we found that the response obtundation was maximum in the 7 minutes group as compared to the 3 and 5minutes group. Thus we conclude that timing of a drug is as important as its dosing.
