Comparison between operative and non-operative management in abdominal injuries

Abstract

Background: Blunt abdominal trauma and penetrating injuries to abdomen is usually not obvious clinically and frequently unreliable. The knowledge in the management of blunt abdominal trauma has progressively increasing. Aim: To compare outcomes between operative and non-operative management in abdominal injuries. Material and Methods: The present study includes 60 patients of blunt and penetrating abdominal injuries. Out of 60 patients, 23 patients with pneumoperitoneum or hemoperitoneum with hemodynamic instability underwent exploratory laparotomy. 37 patients were selected for non-operative management as they were hemodynamically stable. Results: Out of the total 60 cases 7 patients succumbed to their injuries in this study (11.67%). Six patients belonged to operative group 6/23 patients (26.08%). One patient belonged to non-operative group 1/37 patients (2.7%). Conclusion: Non-operative management for blunt abdominal trauma was found to be highly successful and safe in our study. However, management by NOM depends on clinical and hemodynamic stability of the patient, after definitive indications for laparotomy are excluded.
