Comparison of two automated serum 25(OH)D assays – experience of pediatric hospital laboratory participating in DEQAS proficiency testing

Abstract

<b>Introduction. </b>Vitamin D insufficiency coincides with increased risk/progression of several diseases, including cancer, autoimmune diseases, cardiovascular disease and diabetes. The serum 25(OH)D concentration is a reliable biomarker of vitamin D status that due to pleiotropic action of vitamin D recently is more frequently used in everyday clinical practice.<br><b>Aim.</b> The performance of two recently developed automated 25(OH)D Total assays (LIAISON and ELECSYS) was compared using pediatrician samples and results of DEQAS (The International External Quality Assessment Scheme for Vitamin D Metabolities).<br><b>Material and methods.</b> Within-run and between-run imprecision were evaluated by measurements of pediatric samples replicates. Aliquots of 114 randomly selected patients’ samples were measured on LIAISON and ELECSYS platforms to compare the methods. Accuracy was evaluated using DEQAS control samples with DEQAS HPLC and LC-MS data as gold standard.<br><b>Results.</b> Automated 25(OH)D Total assays, LIAISON and ELECSYS demonstrated good intra- and inter-assay precision, with CV<10%. LIAISON assay showed a performance comparable to ELECSYS with R<sup>2 </sup>= 0.8272 and a mean bias -22.5%.Both methods showed good accuracy according to HPLC and LC-MS in DEQAS system, with bias less than -11.6% for LIAISON system and less than -3.1% for ELECSYS system.<br><b>Conclusions. </b>Precision of automated 25(OH)DD Total assays by LIAISON and ELECSYS are very high and comparable and both have required accuracy to identify vitamin deficiency in pediatric samples.
