Condition of gastric and duodenal mucosa in children and teenagers infected with Helicobacter pylori - own experience

Abstract

Introdution: Helicobacter pylori is considered as a main causative factor in gastritis and duodenitis or ulcerative disease. The aim of the study was to analyze macroscopic and histopathological changes in children and adolescents with H. pylori in-fection. Material and methods: Medical records of 495 patients aged 2-18 years with confirmed infection assessed by endoscopy were analysed. Results: Features of inflammation were endoscopically shown in the antrum in 75.6% of patients, in the corpus in 14.3% of patients, erosive gastritis was found in 0.8% of patients, haemorrhagic gastritis in 1.4% of patients, biliary gastritis in 7.7%, polyps of antrum in 1%, granulation of antrum 42.6% and in the corpus of 9.1% of patients. Gastric ulcer was found in 0.2%, duodenal ulcer in 4.4%, erosions of duodenum in 0.8%. Chronic gastritis was histopathologically found in 99.4% and in 0.6% of patients — gastritis reactiva. Degrees of inflammation in the antrum were as follows: mild in 67%, moderate in 19.2%, severe in 6.9% and lack of inflammation in 6.9% of cases. Degrees of inflammation in the corpus were as follows: mild in 10.9%, moderate in 2.2%, severe in 2.0% and lack of inflammation in 84.9% of cases. Degrees of infection in the antrum were as follows: mild in 73.5%, moderate in 14.9%, severe in 4.6% and lack of inflammation in 7%; degrees of infection in corpus: mild in 8.2%, moderate in 2%, severe in 0.6% and lack of inflammation in 89.2% of cases. Pangastris was histopathologically confirmed in 12.9% Conclusions: 1. Gastritis with associated H. pylori infection is usually chronic, with the presence in the antrum. 2. Granulation of gastric mucosa more often exists in the region of antrum and is not always present. 3. Duodenal ulcer is more often shown in association with H. pylori infection.
