The efficacy, tolerance and safety of calcium lactogluconate in the treatment of osteopenia and osteoporosis in the developmental age. Part 2. evaluation of calcium-phosphate metabolism and bone turnover markers

Abstract
The aim of the study was to evaluate the influence of calcium lactogluconate treatment on calcium-phosphate metabolism indices and bone metabolic markers in children with osteoporosis and osteopenia. Material and methods. The study comprised 100 patients aged 5-19 years with low bone mass: in 40 - osteoporosis and in 60 - osteopenia was diagnosed. In the treatment calcium lactogluconate (Calcium 500D preparation) was used. The doses differed depending on diagnosis and progression of bone mineralization decrease. Before and after 12 months of treatment the serum con-centrations and urine elimination of calcium, phosphorus and magnesium ions were performed. The concentrations of parathormon, liver metabolite of vitamin D — 25OHD and bone metabolism markers: collagen type I crosslinked N-teolpeptide (NTX) and bone isoenzyme of alkaline phosphatase were also determined. Results: Statistically significant differences in calcium and magnesium concentrations before and after treatment were found, but they were within normal range. Increase of hypercalciuria was not observed. In children with significant deficiency of vitamin D an increase of 25OHD concentration back to normal range, after treatment, was found. Mild acceleration of bone formation - increase of bone isoenzyme of alkaline phosphatase, in children with osteoporosis was revealed. In all patients a decrease of NTX urine elimination (bone resorption marker) was observed, especially evident in children with osteoporosis. Conclusions 1. Calcium lactogluconate preparation has positive influence on calcium-phosphate metabolism and bone turnover markers in children during intensive growth. 2. Evaluation of bone turnover markers points on, that calcium lactogluconate inhibits bone resorption, which supports physiolo- gical prevalence of bone formation processes. Usage of this preparation in the developmental age could be a part of prevention of perimenopausal and involution osteoporosis.
