CRITERIA FOR THE RETURN TO SPORT TRAINING IN PATIENTS AFTER ANTERIOR CRUCIATE LIGAMENT RECONSTRUCTION (ACLR)

Abstract

The goal of the study was an evaluation of physical fitness in patients after anterior cruciate ligament reconstruction (ACLR) using the Sport-related Determinant of Functional Assessment (SDFA), allowing to make the decision concerning the patient's return to sports training.The study was conducted in 63 patients (group I - 25 females, group II - 38 males) after ACLR. Prior to the surgery, the subjects were regularly involved in different sport disciplines as amateur and professional athletes. The patients underwent four stages of rehabilitation. SDFA was used as a complementary approach to clinical evaluation and the assessment of rehabilitation effects for the needs of sport. Additionally, in the competitive runner selected from the studied sample, an individual SDFA for competitive sports was used, containing a wider and individual range of biomechanical examinations.In both groups the clinical evaluation showed stability of the operated knees. During the 24th week following ACLR the subjects performed treadmill runs, jumps with one leg, running up and down the stairs, alternatingly using the involved and the uninvolved leg. The moments of maximal strength (MMS) were restored in the operated legs and turned out similar to these obtained from the uninvolved legs. In the comparative individual study, the patient, a competitive runner, between the 24th and 27 week of rehabilitation obtained similar values of peak torque (PT) during the isokinetic test and MMS under static conditions (in the sagittal plane) in the operated knee as compared to the uninvolved one. During the 24th week he ran with the speed of 6.67 m/s, covering the distance of 180 m. During the 7th month following ACLR the athlete performed a maximal number of jumps with one leg using the operated leg and obtained ground reaction force values (for the vertical component) similar to these obtained from the uninvolved leg.
