Current status and recommended change in concept of BPP (Foetal BPP (biophysical profile) evaluation by ultrasound

Abstract

Background: There is a need for revaluation of the existing Foetal BPP profiling calculated by ultrasound of the pregnant mother. BPP offers an insight in a nutshell, the foetal activity and allows useful calculations of present and future foetal monitoring, ensuring a healthy child at birth. This study brings forward these flaws in the current methods of BPP assessments worldwide, and also presents a more practical and workable BPP assessment of the foetal parameters. Objective: The objective of the study is to provide the existing methods and their shortcomings of BPP profiling by ultrasound and to provide with alternate BPP profiling method which is more practical in its usage by the Sonologist, ensuring more workable information to the treating Surgeon. Material and Methods: All studies were performed at JNU Medical College & Hospital, Jaipur, in the Radiology department, in pregnancies ranging from 15 to 40 weeks in about 2000 such patients over a period of two years, from November 2015 up to May 2017 after obtaining written informed consent from the patients and prior permission from the ethics committee. Pregnant mothers ranging from 15 to 40 weeks were taken as the study group with no age bar. Each patient was scanned for a minimum of 30-40 minutes at least. Results: The various stages of pregnancies were evaluated by ultrasound for BPP evaluation, and it was found that the BPP methodology proposed by this article ensured a more practical approach to foetal activity measurement as opposed to the existing methods. Conclusion: The study shows that BPP methodology ensured a more practical approach to foetal activity measurement. It also ensured the BPP calculations over a wider range of pregnancy period allowing calculations even before 32 weeks, and permitted more practicale amniotic fluid calculation to be included in the BPP parameters.
