Cytopathological Spectrum of Lesions in Cervical Pap Smears – based on the Bethesda System

Abstract

Introduction: Cervical cancer is the second most common cancer in women, with an estimated 5,30,000 new cases every year. Cervical cytology is the most successful cancer screening technique of the 20th century in the developed countries. Pap smear helps us to identify lesions which can be non neoplastic to lesions with significant epithelial abnormalities. Present study aims to see the cytopathological spectrum of lesions in cervical smear and their incidence with respect to the age group. Methods: This prospective study was conducted from June 2014 to June 2016, by collecting pap smears from 500 women aged between 21-76 years. Collected smears were processed by routine Pap staining techniques. Results: Out of 500 cases, 309 cases (61.8%) were non-neoplastic lesions which comprised 295 cases of inflammatory smears and 14 cases of reactive (atrophic) smears. 65 cases of (13%) epithelial cell abnormality smearswere found. Majority of Inflammatory smears were seen in the age group <50 years old people and majority of the Reactive (atrophic) smears were seen in the age group >50 years. Epithelial abnormalities were more common in >40 years old patients. Conclusion: Cervical Pap smear is effective screening test to analyse the spectrum of cervical lesions. In our country India, though the inflammatory lesions of cervix are more common, pre invasive lesions and neoplastic lesions account for significant numbers due to huge population. Identification of the pre invasive lesions and early stages of cancer of cervix by this screening test can prevent mortality from cancer of cervix.
