ДЕТЕРМІНАНТИ ЗДОРОВ’Я НАСЕЛЕННЯ: СУТЬ ТА ОСОБЛИВОСТІ СИСТЕМАТИЗАЦІЇ

Abstract
Більшість показників стану і динаміки здоров’я населення України протягом останніх десятиліть істотно гірші за європейські, а деякі й за середньосвітові. Тому українська система охорони здоров’я потребує трансформації. Вона має ґрунтуватися на принципово новій інтерпретації причинно- наслідкових зв’язків між навколишнім середовищем (соціальним та фізичним) та здоров’ям населення, забезпечувати формування якісно нових підходів до збереження здоров’я як на індивідуальному, так і популяційному (національному, регіональному) рівнях. Основою для цього має стати дослідження детермінант здоров’я, їх систематизація та структуризація. У статті подано авторське тлумачення поняття «детермінанти здоров’я». Здійснено систематизацію цих детермінант залежно від масштабів їх впливу на здоров’я населення. Виділено три рівні: перший – контекстуальний, детермінанти якого визначають характер детермінант інших рівнів. До цього рівня віднесено глобальні та суспільно-політичні детермінанти (рівень демократії, політична обстановка, рівень корупції, ефективність державного управління). Визначено, що детермінанти другого – структурного – рівня впливають на здоров’я різних груп населення: територіальних, демографічних, соціальних, етнічних і т. п. Детермінанти третьо- го – проксимального рівня обумовлюють індивідуальне здоров’я. Для обох цих рівнів спільними є соціальні, економічні, культурні та екологічні детермінанти, але механізми їхньої дії різні.
