ДИФФЕРЕНЦИРОВАННЫЙ ПОДХОД В ВЫБОРЕ МЕТОДА ГЕПАТОБИЛИАРНОЙ ДЕКОМПРЕССИИ

Abstract
The aim of this study was to identify complications after combined partial endoscopic papilosphincterotomy (EPST) in conjunction with balloon dilation of the major duodenal papilla (MDP) in comparison with standard methods of decompression of the hepatobiliary system. In a prospective study the results of application of various hepatobiliary decompression were performed in 263 patients from 2015 to 2017 years. All patients in the preoperative period applied differential treatment on the basis of the results of impedance spectroscopy, which allows to distinguish the type of morphological changes in the MDP. Balloon dilation was performed to patients with unchanged MDP, partial EPST with balloon dilation to patients with edema of the MDP and full EPST to patients with fibrous stenosis of MDP. A combined methodology of partial EPST with balloon dilation is the method of choice for spasm of MDP and characterized by a small percent (0.8 %) of complications.
