Динаміка інтенсивності втоми у хворих на ХГС після досягнення ними стійкої вірусологічної відповіді та її зв’язок з рівнем кріоглобулінів у сиворотці крові

Abstract
Results published in the article, are continuation of the investigation of prevalence and intensity of fatigue signs in patients with Сhronic Нepatitis C in dynamics a er completion of treatment with medicines possessing direct antiviral action until achievement of stable virological response (SVR). To pursue this goal, 201 patients with Сhronic Нepatitis C out of 214 individuals with con rmed presence of fatigue during the previous examination were involved in the research. By gender, the examined patients were divided as follows: 108 males (53.73%), females – 93 (46.26%), aged from 18 to 65 years. All patients achieved SVR, which was con rmed by PCR method according to clinical protocol. In addition, patients’ age, body mass index (BMI), the level of activity of alanine aminotransferase in blood serum (ALT), level of RNA HCV, genotype of RNA virus, the degree of brosis and activity of necrotizing in ammatory changes in the liver tissue, which had been detected before the onset of treatment, were also considered. As well as in the prior investigation, the presence of fatigue was also established and the degree of severity was determined by means of visual analogue scale. All patients had cryoglobulin level detected by spectrophotometric method. It has been established that a er completion of antiviral therapy and patients’ achievement of SVR, fatigue persisted in more than half of patients (53.23%) and signi cantly worsened their life. As a result of analysis of patient distribution by gender, it has been established that a er completion of treatment and achievement of SVR, fatigue statistically reliably (р>0.05) more o en continued among women – 68 (63.55%) than among men – 39 (36.45%), among elderly individuals irrespective of gender, in patients with HCV genotype 3 and higher body mass index. By the results of assessment by visual analogue scale, mild fatigue was observed in 9 (8.41%), moderate fatigue in 47 (43.93%) and severe fatigue in 51 (47.66%) individuals. In most patients (93%), in whom fatigue persisted a er completion of therapy, cryoglobulins were detected. Correlation between the level of cryoglobulinemia and degree of fatigue manifestation has been established.
