Early results of laparoscopic operations due to colorectal tumours

Abstract
Aim. Assessment of early results of laparoscopic treatment of colorectal tumours.Material. We operated on 208 patients due to malignant and benign colorectal tumours in the period of 2 years. Data were collected prospectivelly. Localisation of tumours, type and time of surgery performed, causes of conversions, histopathology of the tumours, postoperative complications and lenght of postoperative hospital stay were assessed. Results. Laparoscopic operation was performed in 73 patients (35.1%) and opened surgery was carried on 118 patients (56.7%). Conversion rate was 17% – 15 patients of 88 whose operation started laparoscopically. Fifty laparoscopic operations and 6 conversions were performed among colonic tumours patients while 23 laparoscopic and 9 conversions among rectal tumours patients. Average time of laparoscopic colonic surgery was 165 minutes while laparoscopic rectal surgery was 205 minutes. Removed bowel specimens were shorter after laparoscopic than opened operations. Numbers of harvested lymphnodes were higher after opened colorectal operations except of laparoscopic resection of sigmoid colon. Postoperative complications rate was 28.4% (59 patients). Rate of anastomotic leak was 4.1% after laparoscopic operations (3 patients), 3.3% after opened operations (4 patients) and 6,6% after converted operations (1 patient). Average length of hospital stay of uncomplicated patients was 7 days after both laparoscopic and opened operations. Mortality rate of whole group was 4.3% (9 patients). Mortality rate of laparoscopic operations was lower (2.7%) than opened operations (5.9%). Conclusions. 1. Early results of surgical treatment of colorectal tumours confirm beneficial effect of laparoscopic surgery. 2. High advancement of colorectal cancer with infiltration of adjacent organs decreases possibility finishing operation with laparoscopic way.
