Pain in chronic pancreatitis

Abstract

Abdominal pain is the most important clinical sign in chronic pancreatitis (CP) and impairs patients quality of life.The pain can be related to: increased pressure in pancreatic ducts and parenchyma, ischemia, and also pancreatic neuropathy, neuronal remodeling and neuronal plasticity. In addition pain may result from CP complications as: stenosis of the common bile duct, duodenum, pseudocysts, pancreatic carcinoma. Thus pain in chronic pancreatitis is nociceptive and neuropathic.In pain management in chronic pancreatitis the conservative, endoscopic and surgical therapy are used. Conservative treatment include cessation of alcohol and smoking, use of analgesics with adjuvant therapy, pancreatic enzymes, and antioxidant supplementation. In selected group of patients the endoscopic or surgical therapy are used (mainly decompression procedures, less commonly partial pancreatic resection). Celiac plexus blockade or neurolisis, thoracoscopic splanchnicectomy, and rarely total pancreatectomy are also performed.
